Initial evaluation for new clients is billed as CPT code 99205 for 60-74 minutes, follow up 40-54 minutes visit is billed as 99215 and 30-39 minutes visit is billed as 99214. My current charge for the CPT code 99205 is $245 and for 99215 the charge is $200. 

*Note that even if appointment lasts 90 minutes I charge based on a total time spent on the visit (additional time before visit to review documentation and after visit to document, care coordination, review of labs or medical records, etc.)

There are add on codes for prolonged and additional services: 99417, G2212, 96112, 96113, 90885, 90846, 96160, 96130, 96131, 90792, 90833, 90836, 90837, 90834 and 99367. 

My current charge for each unit of 99417 (each additional 15 minutes on top of 99205 or 99215) is $40. Insurance may have a lower allowed amount for this code. If this is the case, I can only charge you whatever allowed amount is (if you are using insurance).

Information About Autism Evaluation Process and Pricing 

I typically require 3–5 visits to complete a comprehensive autism evaluation. Here’s a brief overview of how the process works:

Evaluation Process:

• The initial visit (90 minutes) focuses on gathering medical and developmental history.

• In the follow-up visit(s), I continue interviewing the caregiver, review developmental and school records, conduct behavioral observation, and ask clarifying questions. These sessions help differentiate autism symptoms from other neurodevelopmental or mental health conditions.

• If screening results are unclear, I may ask for additional screening tools. If these remain inconclusive, I may recommend ADOS-2 testing, which would be a separate visit.

 Billing and Insurance:

• The initial visit is typically billed as CPT 99205, plus CPT 99417 for every 15 minutes beyond the standard time (beyond 54 minutes).

• Follow-up visits are usually billed as CPT 99215, also with 99417 units if prolonged time applies.

  • Note: Some insurance plans cap 99417 reimbursement at 3 or 4 units per day.

• Report writing (including interpretation of screening tools, interviews, and records) is billed separately as:

  • CPT 96130 (first hour, $130)

  • CPT 96131 (each additional hour, $130)

  • Report writing usually takes 2–4 hours, depending on complexity and the amount of data reviewed.

 Important Insurance Reminders:

• I strongly recommend contacting your insurance company to confirm:

  • Whether they require ADOS-2 testing in order to approve an autism diagnosis (especially if you’re seeking coverage for ABA therapy)

  • Whether you have a deductible or just a co-pay for these types of services

  • What the allowable amount is for each CPT code

 For example:
I may submit a charge of $350 for CPT 99205 + 99417 x3 units, but your insurance may only allow $290. If you have a deductible, you will be responsible for the allowed amount, not the full charge. If you’ve already met your deductible, you may only owe a co-pay or co-insurance.

For private pay clients my current fees are as follow:

Initial psychiatric evaluation including for Autism/ADHD evaluation (60-90 minutes): $300-$350

Follow up Autism/ADHD evaluation visit (60 minutes): $300 

Feedback and recommendations planning visit for Autism/ADHD (60 minutes): $250

Follow up visit (60 minutes): $250

Follow up visit (45 minutes): $200

Follow up visit (30 minutes): $150

*Fees subject to change and depend on time spent. If the follow up visits are longer than 60 minutes, each additional 15 minutes may be billed at the rate of $50 for each additional 15 minutes increment. Autism and ADHD evaluation may require few visits to complete, depending on complexity. Expect 3 visits on average and 1 follow up/feedback visit. ADOS-2 testing will require additional visit. 

OTHER FEES

*Services provided outside of the usual appointment time, including telephone conversations lasting longer than 15 minutes per week, preparation of documents, care co-ordination, or extensive interactions with insurance companies will be billed at a pro-rate of $200 per hour.  

*Late Cancellation/No Show Fee is $150. If you fail to show up for your appointment or do not notify me in advance, please note that you will be charged this standard fee for the missed session. To avoid the fee, cancellations or rescheduling must be made with at least 24 hours' notice, allowing me to offer the time slot to someone on my waiting list. This fee is not covered by insurance and it applies to every patient. 

*I accept most major credit cards, you can pay via patient portal using Practice Q Payments through Patient Portal or via Chase or Square Pay.
