Mindful Minds offers comprehensive private-pay autism evaluations for children and adolescents younger than 18 years of age. Each evaluation is individualized and designed to determine whether Autism Spectrum Disorder (ASD) best explains a child's developmental, behavioral, social communication, and emotional presentation while carefully considering other conditions that commonly overlap with or resemble autism.
Evaluations are performed by Aneta B. Kucharczyk-Swamy, MSN, PMHNP-BC, CPNP-PC, a dual-certified Pediatric Nurse Practitioner and Psychiatric Mental Health Nurse Practitioner with specialized training in autism assessment, including ADOS-2 administration and Autism Center of Excellence certification. This unique combination of pediatric and psychiatric expertise allows for a comprehensive assessment of developmental, behavioral, emotional, and medical factors that may influence diagnosis and treatment recommendations.
Comprehensive Evaluation Fee: $2,995
This evaluation is recommended for children who have not previously undergone a comprehensive autism evaluation or when there are concerns about multiple possible diagnoses.
The purpose of this evaluation is to develop a comprehensive understanding of your child's strengths, challenges, and diagnostic profile rather than simply determining whether autism is present.
Comprehensive developmental, medical, psychiatric, educational, and family history
Parent and child clinical interviews
Review of medical, educational, and therapy records
Review and interpretation of standardized questionnaires completed by parents, teachers, and the child (when appropriate)
Clinical observation and behavioral assessment
Standardized autism assessment measures, adaptive functioning assessment, cognitive screening, and other evidence-based assessment tools selected based on clinical judgment
Comprehensive differential diagnosis, including consideration of ADHD, anxiety disorders, OCD, mood disorders, trauma-related conditions, learning concerns, and other neurodevelopmental or psychiatric conditions when clinically indicated
Comprehensive written diagnostic report
Feedback appointment with individualized recommendations for treatment, school supports, accommodations, and community resources
Assessment tools are selected based on each child's individual presentation and may vary from one evaluation to another.
Comprehensive Evaluation Fee: $2,295
This evaluation is intended for children who already have an established diagnosis (such as ADHD, anxiety, OCD, depression, or another psychiatric condition) and whose primary concern is determining whether they also meet diagnostic criteria for Autism Spectrum Disorder.
The evaluation focuses specifically on diagnostic clarification regarding autism while reviewing existing diagnoses as part of the differential diagnosis.
The evaluation includes the same comprehensive diagnostic process, but is more focused and tailored to the referral question.
If additional concerns arise during the evaluation that extend beyond the original referral, additional assessment or referral for further testing may be recommended.
Every evaluation is individualized. Assessment measures are selected based on clinical judgment and the child's unique developmental profile. Not every child requires the same testing.
Depending on the clinical presentation, the evaluation may include standardized autism assessment measures, adaptive functioning assessment, cognitive screening, behavioral observations, and other evidence-based assessment tools.
For children with very complex developmental presentations, suspected intellectual disability, learning disorders, significant cognitive concerns, or diagnostic uncertainty, I may recommend a comprehensive neuropsychological evaluation or formal standardized cognitive (IQ) testing performed by a pediatric psychologist or neuropsychologist.
When clinically appropriate, I may administer the Kaufman Brief Intelligence Test, Second Edition (KBIT-2) as a brief cognitive screening measure and the Vineland Adaptive Behavior Scales to assess adaptive functioning. These assessments provide valuable clinical information but do not replace a comprehensive neuropsychological evaluation or full standardized IQ testing.
A 25% non-refundable deposit is required at the time of scheduling to reserve evaluation appointments.
The remaining balance is due before or at the first evaluation appointment, unless other arrangements have been made in advance.
Because autism evaluations require significant scheduling time and preparation, the deposit is non-refundable. If an appointment is rescheduled in accordance with the practice cancellation policy, the deposit may be applied to the rescheduled evaluation.
The evaluation fee is all-inclusive and covers the complete diagnostic process, including:
Clinical interview(s)
Review of records
Review and interpretation of questionnaires
Selection and interpretation of assessment measures
Diagnostic formulation
Comprehensive written report
Feedback appointment
Individualized recommendations
There are no additional charges for these components when they are part of the evaluation.
These evaluations are offered as self-pay services.
Upon request, I will provide a superbill that you may submit to your insurance company for possible out-of-network reimbursement. Reimbursement is not guaranteed and depends entirely on your individual insurance plan and benefits.
