Hybrid Care Model
At Mindful Minds, LLC, we offer both secure in‑person sessions at our Seattle office and HIPAA‑compliant video telehealth throughout Washington State. You can choose whichever works best—based on your preferences, clinical needs, and convenience.
Patients who are seen via telehealth appointments must be physically located in the state of Washington during time of service.
At Mindful Minds, LLC, my goal is to create a thoughtful, comfortable, and collaborative space where you feel truly understood and supported. I take a whole-person approach that blends medical knowledge, clinical experience, and genuine curiosity about your story.
I provide psychiatric evaluations and diagnostic assessments for ADHD and autism. However, I do not offer testing for dyslexia or other learning disorders — those evaluations are completed by licensed psychologists who specialize in learning assessments. I do not offer neuropsychological testing.
Once you schedule an appointment, you’ll receive an intake packet and consent forms to complete electronically. If we’re exploring a specific diagnosis, you’ll also receive additional screening tools and questionnaires. I use several secure, third-party platforms such as WPS Publishing, Pearson Assessments, MultiHealthSystems (MHS), and NovoPsych.
Please know that an evaluation is a collaborative process — not something that’s simply “done to you.” You’ll play an active role in helping me understand your background, symptoms, and goals. This may include:
Completing questionnaires and forms carefully.
Providing school records, prior evaluations, or family feedback when helpful.
Gathering relevant medical or psychological records for review.
It’s important to allow yourself time and space for this process — it’s an investment in your own insight and growth.
All intake paperwork should be completed at least 72 hours (and no later than 48 hours) before your appointment otherwise your appointment will be cancelled and you will need to contact me to reschedule.
Your first visit will focus on getting to know you — learning about your history, your current concerns, and what brings you in. If you’re seeking a new evaluation, please know that it may take more than one visit to complete the full process unless your case is very straightforward.
This step-by-step approach helps ensure we understand the full picture and create a plan that feels accurate, thoughtful, and genuinely helpful.

It’s your responsibility to verify whether I am in-network with your insurance plan and to understand any out-of-pocket costs you may be responsible for.
Please note that I’m unable to schedule or provide services without your current medical insurance information.

Please also read cancellation policy and fees for late cancellations:
Late Cancellation/No Show Fee is $150. If you fail to show up for your appointment or do not notify me in advance, please note that you will be charged this standard fee for the missed session. To avoid the fee, cancellations or rescheduling must be made with at least 24 hours' notice, allowing me to offer the time slot to someone on my waiting list. This fee is not covered by insurance and it applies to every patient.
If you have Medicaid insurance, I am not permitted to charge a late cancellation fee. However, my policy allows for one late cancellation as a courtesy. If there are two late cancellations, I may need to discontinue services so that I can make space for other clients who are waiting for care.
